The VA is one of the most complex payers in healthcare. Strict deadlines, detailed rules, a payment posture that isn't forgiving. With VA enrollment up 30% since the PACT Act and now exceeding 9.1 million veterans, the volume of claims subject to this rule is only growing. When a veteran receives emergency care at a non-VA facility, the VA must be notified within 72 hours. Miss that window and the claim is denied, regardless of medical necessity. It sounds manageable. May 202611.81% - that's the healthcare initial claim denial rate in 2024. In 2022, 10% of claims were denied. By 2025, that number is 41%. A 310% increase in 3 years. And it's not slowing down. Prior auth denials dropped 7.7% which looked like progress. But medical necessity denials rose 5% over the same period. Requests for additional information jumped 5.4%. The scrutiny didn't go away. It just moved to the back end. Meanwhile, providers are collecting $3 less per $100 billed. AR days are up 5%. Collection rates dropped from 37.6% to 34.5%.
