May 2026A conversation with Carl June on what’s next for cell therapy Great to catch up again with Carl—one of the true pioneers behind CAR-T—during a recent discussion. Carl’s continued influence is unmistakable—recently named to the 2026 Forbes America’s Greatest Innovators list. In our conversation, we explored a question that’s top of mind for many of us: What does the rise of in vivo CAR technologies mean for the future of cell therapy? A few takeaways that stood out: • This is expansion, not replacement. In vivo CAR (LNPs, viral delivery) adds a powerful new modality—but ex vivo approaches aren’t going anywhere. Their ability to support deep engineering (gene edits, multiplexing) will remain critical, especially in solid tumors. • Different diseases, different “sweet spots.” Oncology, autoimmune disease, and even regenerative medicine will each favor different platforms depending on risk tolerance, delivery, and mechanism. • Autoimmune is the surprising frontier. Perhaps the most striking shift: CAR-T moving into autoimmune disease—with lower toxicity than expected and rapid clinical adoption. This wasn’t even feasible to trial in the U.S. just a few years ago. • The immune system is an orchestra. Beyond T cells, we’re seeing momentum across NK cells, macrophages, Tregs, and γδ T cells—each with distinct advantages in persistence, targeting, and scalability. • The “body as a bioreactor.” In vivo delivery opens the door to treating patients on demand, potentially transforming access and logistics. As the field moves away from a single dominant approach, CellCarta is building a comprehensive toolbox of modalities—because the future lies in precisely matching each tool to each disease.
