May 2026CMV anterior uveitis — underdiagnosed and, in Ontario, undertreated. Until now. Recurrent hypertensive iritis, coin-shaped KPs, IOP spikes that don't behave like herpes? Think CMV. Aqueous PCR is the diagnostic key. The treatment gap in Ontario has been real: 🔴 Ganciclovir 0.15% gel — insufficient concentration for anterior segment disease 🔴 Oral valganciclovir — effective, but no LU code for uveitis in Ontario. Out-of-pocket cost: ~$1,300/mo for induction, ~$700/mo for maintenance. Not sustainable for most patients. Since joining the University of Toronto Uveitis Service, we addressed this: we now compound ganciclovir 2% eye drops through our pharmacy. The evidence across 7 studies (2017–2025): ✅ 87–100% clinical response ✅ IOP and KP improvement within the first week ✅ No systemic side effects ✅ Recurrence manageable with slow taper and maintenance ≥4x/day If you're seeing recurrent hypertensive AU, Posner-Schlossman-like presentations, or iritis refractory to standard therapy, consider an aqueous tap. Apr 2026Sarcoidosis work-up in uveitis — a quick update on two tests we're rethinking in our Toronto clinics. ACE → sIL-2R (soluble IL-2 Receptor) Serum ACE has limited sensitivity (~60%) and specificity. sIL-2R has shown better diagnostic performance for ocular sarcoidosis — higher sensitivity and useful as a marker of disease activity.(Groen-Hakan et al., JAMA Ophthalmol 2017; Febvay et al., Br J Ophthalmol 2015) CXR → Chest CT Chest X-ray misses a meaningful proportion of hilar/mediastinal lymphadenopathy and parenchymal involvement. For patients with suggestive uveitis phenotype, low-dose chest CT is more sensitive and is now the preferred imaging. (Kaiser et al., Ophthalmology 2002; Takase et al., Br J Ophthalmol 2013) In our Toronto clinics we're standardizing the work-up around sIL-2R + chest CT when sarcoidosis is on the differential. #Uveitis #Sarcoidosis #Ophthalmology #Toronto #EvidenceBased
