Why is Instacart joining a rural health transformation collaborative? Because the hardest part of chronic disease management is often not writing the care plan. It is making the care plan executable. Today, we announced that Instacart has joined the Collaborative for Healthy Rural America, which I am proud to have helped convene last fall to help states move from Rural Health Transformation planning to real implementation. When I first met Sarah Mastrorocco at JPM in early 2026, her and her team's passion for leveraging their powerful platform to bring food for health infrastructure to support real people across the United States stood out to me. The stories and proven examples Sarah and her team at Instacart brought to bear resonated with me, our CHRA partners and the outcomes we hope to help States and rural health providers achieve. That distinction matters. States do not need another collection of point solutions. They need an operating model. One that can connect longitudinal primary care, AI-enabled population management, virtual access, interoperable data, closed-loop referrals, community services, and now more direct access to nutritious food and everyday essentials. Food access is not a side issue in rural health. For many patients managing diabetes, maternal health, heart disease, obesity, COPD, or other chronic conditions, nutrition is part of the intervention. But in rural communities, the gap between a clinical recommendation and a patient’s daily reality can be measured in miles, transportation constraints and local availability of resources. That is why Instacart ’s role is so important. With Deloitte , Lumeris , Teladoc Health , Unite Us , Nuna and now Instacart , CHRA brings scaled, complementary capabilities to states, providers, and communities trying to turn federal investment into measurable outcomes. This is not about building a bigger vendor ecosystem. It is about reducing fragmentation. It is about helping states move faster without forcing every health system, rural provider, or agency to assemble the same capabilities from scratch. The next phase of rural health transformation will be judged by execution. Not intent. Not pilots. Not disconnected innovation. Execution.
