*Inflammatory markers are moving from exploratory to primary* Two years ago, if a sponsor put hs-CRP or Pro-C3 into an obesity study, it was in the exploratory biomarker panel. Nobody expected the signal to matter for the primary story. That has changed faster than most teams expected. In the last twelve months, we've watched sponsors move inflammatory and fibrotic markers into the key secondary tier. A few have written them into primary endpoints for Phase IIb programs with a metabolic-steatotic indication. The reason is population overlap. A meaningful share of patients enrolled in obesity studies also carry MASLD, and most are undiagnosed at screening. The hepatic and inflammatory effects of GLP-1 therapy, favorable and unfavorable, are showing up in the data whether or not the protocol was designed to capture them. Sponsors who are not measuring are reading competitor papers and realizing they missed the story. Moving a biomarker from exploratory to primary means central lab validation, disciplined sample handling, a statistical analysis plan with real pre-specification, and enough retained samples to run confirmatory analyses. None of that is inexpensive. All of it is less expensive than running a second study. My read for metabolic sponsors: assume some fraction of your obesity population has silent inflammatory or hepatic disease. Design the biomarker panel so the answer is in your hands when the data reads out. More in the full paper next week.
